This form is always filed under the bride's name. The first three sections (Bride's Information , Groom's Information , and Information for the Couple Together ) are normally filled out by the bride, though sometimes it may be filled out by the groom, the marriage preparation coordinator, or the director of adult faith formation. If the couple is being prepared elsewhere, this segment will be filled out by the wedding presider or the pastor (or his delegate). If you are the bride or the groom, please do not fill out any more than these first three sections.

This section is normally to be filled out by the marriage preparation coordinator or the director of adult faith formation (or his delegate).
Normally to be filled out by the preparing priest or deacon, and/or the director of adult faith formation (or his delegate).
Normally to be filled out by the marriage preparation coordinator, the classroom teacher, or the director of adult faith formation (or his delegate). " Couple to couple" is when a sponsor couple works with one other couple rather than having the couple preparing for marriage take formal classes; checking that box indicates that there will only be one ending dates (rather than four), as the class schedule may not be followed exactly. In the case of couple-to-couple, only one class date box needs to be filled out.
Normally to be filled out by the preparing priest or deacon. If the couple is being prepared elsewhere and they are having their wedding at St. Luke, the letter that the preparing clergyman sends to St. Luke to affirm that the couple is ready to set a date would be used as the basis for the wedding presider to fill out this segment. If the couple wants to bring in a presider from outside the parish, the pastor needs to be involved; no potential outside presider may preside without the pastor's explicit approval.
When it is appropriate to set a date for the wedding, please complete a peach wedding-date form and give it to the couple so that they can take it to the Office of Worship or the evening receptionist.. The peach form requires the presider's signature, not the pastor's (unless the presider is coming from another parish, then the pastor would sign it). Once this form reaches the Office of Worship, the Office of Worship can begin setting a wedding date. Please direct the couple to contact the Office of Worship to plan their wedding liturgy. When the clergy-meeting section is complete, the director of adult faith formation will know that he may print a Twogether-in-Texas certificate for the couple; he will not look for the completion of this section on his own but will only print the certificate when the couple or the preparer asks him to print it. This certificate allows a discount from the cost of their marriage license, because they completed a state-recognized marriage preparation program. If the couple was prepared elsewhere, that parish should print the certificate. If the couple is already civilly married or is getting married outside of Texas, the Twogether in Texas certificate will have no value for them.
